Tracey’s story – finding freedom and independence
Tracey is a 37-year-old woman who has faced many
challenges in her life. After experiencing trauma and
moving through several placements which did not meet
her needs, Tracey faced growing restrictions which
limited her independence and freedom to move around.
By 2018, she was living in a service where alarms were
fitted to her doors; outings were rare and only possible
with two staff by her side. Tracey spent most of her time
inside her flat, and struggled with her health. A lack of
structure around meals contributed to weight gain and
low confidence.
Since joining Active Prospects, Tracey’s life has
transformed. She has flourished into a fun-loving,
vibrant woman with a great sense of humour and a
passion for life. She now enjoys the freedom to come
and go from her flat as she chooses. Out in her local
town, she now needs only 1:1 support, and has grown
in confidence using public transport and making
connections with other people she meets along the way.

Tracey has embraced new routines and a healthier
lifestyle, following a meal plan that supports her
wellbeing. She is much more sociable, enjoying time
with her housemates and taking part in group activities.
At a local day centre, she has discovered a love
for cooking and pottery, making new friends
through shared activities and creativity. In her
free time, she delights in shopping for jewellery,
glittery accessories and bags, flipping through
magazines, and dancing to her favourite music.
Tracey is no longer defined
by restrictions or risks but by
her laughter, friendships,
and growing independence.
With the right support, she is
enjoying life on her own terms.
